It's Form Season Again!
For patients who have had a Well Visit within the last year, any required school, camp, and other forms can be dropped off, faxed, mailed, or sent through the patient portal attached to a message to the doctor. If forms are faxed or sent through the portal, please call to confirm they have been received. There is a $10 form fee per form, and please allow a 5 business day turn-around for completion. We cannot accept emailed forms or return them via email due to HIPAA encryption requirements. We will not mail forms unless a self-addressed, stamped envelope is provided. We currently are unable to return forms through the portal and are sorry for the inconvenience. PLEASE make sure the patient information is completed on all pages and the parent section is completed before submitting the forms to us. You will be notified when they are completed, but please let us know if you don't receive a call so we may return the forms to you in a timely manner. Expedited forms (needed within 24 hours) may be subject to an additional fee.
Online Scheduling
Online scheduling is available for *existing patients only.* A limited number of appointments are available online so please call the office for additional dates and times. When scheduling online, if prompted to enter your address and additional demographic information you are not scheduling correctly. Entering demographic information will create a duplicate chart.

Missed Appointments
We understand things come up and sometimes patients are unable to make it to their appointment. Please cancel appointments 24 hours prior to the appointment time. No shows will be subject to a $50 fee.
